***NOTE - The following rules may not always be the case i.e. there may be special instances where a genin for example may do a B ranked mission and therefore be required to do less overall missions. Although this would only happen on special occaisions and would be very rare.
The following are the ranks in order of lowest to Highest:
Academy Students
Genin - (low ninja)
Chuunin - (middle ninja)
<Oinin -(hunter ninja)
<Tokubetsu Jounin - (special high ninja)
<Jounin - (High ninja)
ANBU (Black Ops) - (Ansatsu Senjutsu Tokushu Butai - Assassination Tactics Special Squad)
Oumyounin - (Secret Ninja, added in place of Sannin)
Kage - (Shadow) - Most powerful and highest ranked, head of the hidden village.

And so on, there will most likely be 1 or 2 additions to this list, specifically another type of specialized squad.
How do I go up in rank ? - For Academy Students, Genin and Chuunin the only way to go up in rank is pass an examination. Everythign else revolves around experience and skill level.
Keep in mind that there are always special exceptions, the mission numbers are generally just a guide to ensure enough experience is gained however if a Ninja progresses enough they may "skip ahead" so to speak doing higher level missions, or gaining ranks faster or receiving special privilages.
Academy Students - After you have learned the basic skills and completed your tests all Academy students are split into groups of three. Each group is assigned to a Jounin who will test and exam them and decide whether or not they can become Genin or return to the Academy. Students have special events and tests and may do missions at E rank, however they do not qualify as actual "Ninja" rank missions. Students must also complete 5 missions of the following rank:
Genin - Once you pass the first exam you become a Genin (low ninja) and are now considered a shinobi. The Jounin (usually the one who passed you) will be in charge of your training and taking you on missions. After Genin is Chuunin to become Chuunin you must also take an exam, however this exam must be done in groups of three. Another requirement is a Jounin must reccomend a group for the exam, you cannot just take the exam if you want to. Genin are able to do missions as high as C rank. Genin must also complete 25 missions of the following Rank:
Chuunin - Having passed the chuunin exam you are now a Chuunin (middle ninja), and are able to lead teams of genin on missions. Chuunin are also responsible for teaching at the Academy of their village. Chuunin must train and teach students, but they also have 3 choices of what they want to become. Chuunin are able to do missions as high a B rank. Chuunin must also complete 50 missions of the following Rank:
Hunter Ninja or Shinobi Hunters - A chuunin must apply for this postion and receive special training. Hunter Nins spend a lot of time in other countries looking for Missing nin or Documents. Hunter Nin are considered to be lower than Jounin. Hunters receive special missions.
Tokubetsu or "Specialized" Jounin - A specialized Jounin is a Jounin that, rather than taking the Jounin exam, has decided to study only one area. An example of a Special Jounin is one that studies Healing Techniques. Because these Ninja specialize in one are, they are considered to be lower than Jounin. These Jounin are often put into teams of other FULL Jounin and sent on missions. These Jounin are trained by either Sannin or the Kage of their villages in special and sometimes Forbidden techniques.
Jounin - This is the next 'official' rank after Chuunin it requires a special exam. Jounin receive training in all areas rather than specific training like Tokubetsu Jounin or Hunters and are considered to be above them because they have a wider variety of training. Jounin are in charge of Genin exams and training Genin. Jounin also peform most of a villages missions espcially the higher ranked ones. Jounin are also required to complete 100 missions of the following Rank:
they are also responsible for training genin and taking them on missions.
ANBU (Black Ops) - Is a special squad in charge of investigating and protecting the village they live in. They may also be sent of special high ranked missions and dangerous assassinations To become a member of ANBU a Jounin must first apply and then be Approved by the Kage of the village they live in. For ANBU to become Oumyounin they must be approved by the Kage. They must also have first fulfilled the following requirements.
Missions:
Train 6 Ninjas of the following Rank
2 Jounin
4 Hunter Nin
Other
Show great loyalty and Pride to their Village
Oumyounin - To become a Oumyounin or "Secret Ninja" you must first become a member of ANBU and prove your loyalty and skills. Only the Kage of the village may decide whether or not to promote you to Oumyounin, however those at the Oumyounin level receive special training from the Kage and learn VERY powerful S ranked and Forbidden techniques.
Kage - The head of the village, usually the most powerful Ninja in the village. There can only be one Kage at a time.
